Peptic ulcer and chronic renal failure.

چکیده

PROBABLY the first real attempt to ascertain if any casual relationship existed between peptic ulcer and renal disease was contained in a survey of post mortem records by Perry and Shaw (1893). They found that some form of kidney disease was present in seven per cent of deaths and as their cases of duodenal ulcer numbered 70, they reasoned that seven per cent, or five cases, might have been expected to have coincident Bright's disease. In fact, 12 cases showed this association and they concluded that "there appears to be some reason for including Bright's disease as one of the predisposing causes of duodenal ulcer". Moynihan in his classical textbook on duodenal ulcer (1910) described 27 autopsy cases where ulceration affecting the duodenum was associated with an advanced pathological condition of the kidneys, and he hinted at an association between the two diseases. Thereafter the subject received little further attention until the early 1960s when attempts were made to prolong the lives of uraemic patients by regular dialysis and transplantation. Subsequently, peptic ulcer was recognised as a serious complication in uraemic patients receiving regular dialysis (Sokol, 1964), and in those undergoing kidney transplantation (Moore and Hume, 1969). In some centres, the incidence of upper gastrointestinal haemorhage and perforation following transplantation was so high it was even suggested that all uraemic patients should have vagotomy carried out prior to transplantation (Penn et al., 1968).

[Guidelines of treatment for peptic ulcer disease in special conditions].

The pathogenesis, incidence, complication rates, response to acid suppression and Helicobacter pylori (H. pylori) eradication therapy in peptic ulcer associated with chronic disease such as liver cirrhosis, chronic renal failure, diabetes mellitus, and critically ill conditions are different from those with general population, so that the management strategies also should be differentiated. The...

Helicobacter pylori infection, chronic kidney disease, and peptic ulcer disease.

The recent publication of “Helicobacter pylori infection rate in chronic kidney disease and end-stage renal disease patients with peptic ulcer disease” is very interesting. Chang and Hu noted that “the H. pylori infection rate is lower in PUD patients with CKD and ESRD than in those without CKD.” There are many concerns regarding this report. First, the prevalence of peptic ulcer disease (PUD) ...

Gastric acid secretion in chronic renal failure.

Contrary to the small amount of published evidence, but in accordance with clinical impression, we have found an increased incidence of peptic ulceration in people with chronic renal failure. Hyperacidic secretion in response to a standard pentagastrin test occurs in patients established on long-term dialysis treatment. The traditional liability of azotaemic patients to peptic ulceration seems ...

Evaluation of frequency polymorphism Interleukin 1B gene in patients with peptic ulcer and chronic gastritis

Introduction: Helicobacter pylori is the main cause of chronic inflammation and peptic ulcer. More than half of the world population is afflicted with Helicobacter, while only 20% of people show the clinical symptoms of the disease. Genes encoding the cytokines interleukin (IL)-1B play a key role in the inflammatory responses of gastric mucus. This study aims to analyze polymorphism in the IL-1...

The frequency of interleukin-10 (IL-10) polymorphisms in patients with peptic ulcer and chronic gastritis

ABSTRACT Introduction: Peptic ulcer is a common problem in medicine with serious impacts on the quality of life of patients. It has been shown that Helicobacter pylori infection is related with inflammatory responses of gastric mucosa. However, some patients remain asymptomatic. Sustained colonization and chronic inflammation increase the risk of gastritis and peptic ulcer. The a...

A case of bismuth intoxication with irreversible renal damage

Bismuth is a chemical element symbolized as Bi and is a trivalent poor metal, which chemically resembles arsenic and antimony. Colloidal bismuth subcitrate (CBS) and bismuth subsalicylate are the bismuth salts widely used in the treatment of peptic ulcers, functional dyspepsia, and chronic gastritis. Intoxications with CBS are rare. In a few case reports, acute renal failure was described, but ...
